Things to Bring
- Insurance Card
- Patient’s ID card (If the patient is a minor, parent’s or legal guardian’s ID is required)
- If you use glasses or contact lenses, please bring them for testing. (If you’d like contact lens prescriptions, please bring the contact lens boxes for both eyes)
- Sunglasses since your eyes will be dilated. (We can also provide you with temporary shades.)
- Bottles of eye drops you are currently using.
- List of medications that you are taking.
